Why Custom Review Forms Matter
Custom Review Forms are a powerful tool to gather product-specific data that can improve your products and help you better understand customer preferences. By tailoring review request emails to specific products or product groups, you can collect targeted feedback that’s more actionable for your business.
New Product Launches: Use custom forms to gather detailed feedback on newly launched products. For example, ask specific questions to gauge customer satisfaction with product performance, design, and features.
Product Categories with Unique Features: For certain categories, like apparel, you can use specific questions (e.g., Fit) to understand how well products suit your customers. Merchants in other industries can ask about specific aspects like durability or comfort.
Multi-Function Products: For products with multiple features, like a dog collar with battery life and comfort attributes, you can gather feedback on each aspect. This enables you to refine products based on how customers experience each feature.

Product-Specific Forms
A Product-Specific Form is a custom form tied to a product or group of products via tags. Tags associate a custom form with specific products, ensuring that the review requests for those products include the custom questions you’ve set up.
Only one tag can be assigned to a product at a time.
To assign a tag to a product:
Click Catalog
Click Product Library
Go to the Product Tags tab > Add Tag or select a product in the library and click "Add Tag" button if the tag is already created
From the Product Tags tab, you can create new tags or edit existing tags to apply to your products. Learn more here.
Getting Started with Custom Review Forms
To create or edit Custom Review Forms, go to Campaigns > Email & Survey Campaigns. On the My Campaigns page, scroll down till you find the section called "Product-Specific Forms" and click "Add New".
From here you will include the following information to create your template:
Form Name: This is for internal use only and won’t be visible to customers.
Product Tags: Assign tags to products so that this form is used for reviews of those products.
Email Customization: Customize the email text that accompanies the review request.
Add a Question: Add Product and Customer question types with unique answers to each after clicking this button. More on this below.
Note:
You can apply multiple tags to a single form/template. The branding elements for the form are the same elements you have set-up in your Default Product Review Request campaign. Learn more about branding elements here.
Question Types
There are two main types of questions you can include:
Customer Questions: These are questions about the reviewer, such as their age, gender, or preferences.
Product Questions: These are questions about the product being reviewed, such as how well it fits, its taste, or durability.
You can make questions mandatory to ensure the question is answered before submitting the review and mark answers as private so they do not display along with the review.
Answer Types
Several types of answers can be used for your custom questions:
Single Choice: Radio button format, where customers can select only one answer.
Multiple Choice: Checkbox format, where customers can select multiple answers.
Rating (1-5): Customers rate a product on a scale of 1 to 5.
Fit: Customers answer how well a product (like apparel or shoes) fits them: too small, true to size, or too big.
Free Text: A text box where customers can provide a detailed response.
Product Net Promoter Score (NPS®): Customers rate how likely they are to recommend your product to others.
Question & Answers Customization
For each question type, you can further customize the appearance and content, including:
Display Title: The title that appears above the question.
Question Text: The specific question being asked.
Guidelines Text: Optional text for Free Text questions to guide customers on how to answer.
Bar Colors: Customize the empty and full bar colors for Rating and Fit answers.
Editing Existing Forms
To manage your templates/forms, go to the My Campaigns page here, finding the Product Specific Forms section and click on the form you want to edit.
You are also able to Enable/Disable and Delete forms.
Note:
When Enabling a form, only that review request template will be used when sending product review requests for products with the specific tag on that form. When Disabling a form, those products will use the Default Product Review Request template.
